When you enroll in the Whitening for Life™ at this cleaning appointment, we will provide you with one set of custom bleaching trays* and whitening gel for a minimal enrollment fee of $99.00. At each 6 month cleaning or 3 month SPT appointment you will receive a Free syringe of whitening gel. This ensures that you will be able to keep your teeth bright and beautiful for life!

All we ask in return is:

- You come to our office for your periodic oral examination and 6 month cleaning appointment or for your Supportive Periodontal Therapy appointment. We must see you for your 6 month cleaning appointment every 6-7 months. If you are a periodontal patient we must see you once every 12-16 weeks (or as often recommended) for your SPT appointment. Your long term dental health is as important to us as it is to you. That is why we are happy to provide this extra bonus for our patients who are committed to their dental health.
- Provide at least 2 business days notice if you need to cancel or change an appointment. In order to provide exceptional services like Whitening for Life™ to all of our patients, we ask that you give us the courtesy of advance notice for schedule changes.
